What we collect.
When you request a call, we collect what you type into the form: your name, phone number, email address, and what you tell us about your situation. If you use the interactive tools on this site, your selections may be stored briefly in your browser and submitted anonymously so we can understand what visitors are looking for.
Why we collect it.
To call you back, to schedule your visit, and to communicate about your care. If you check the text-message consent box, we use your phone number to send appointment-related texts. Message and data rates may apply, and you can reply STOP at any time to opt out.
What we never do.
We never sell your information. We never share it with advertisers.
Service providers.
Form submissions are processed by our website hosting provider and delivered to our practice-management systems so we can respond to you. Live review content is displayed from Google via an embedded widget; interacting with it is subject to Google’s own privacy terms. The map on our contact section is embedded from Google Maps.
Your health information.
Once you become a patient, your health information is protected under HIPAA and our Notice of Privacy Practices. This website is not intended for submitting detailed medical records; please save the full history for your evaluation, where it belongs.
Retention and access.
We keep form submissions as long as needed to respond to you and to meet our legal obligations. You may ask us what we have on file for you, ask us to correct it, or ask us to delete it where the law allows. Call (843) 595-1055 or email info@altrperformance.com and a human will handle it.
